Developers: 

Combines the Self-Efficacy for Healthy Eating scale from the GEMS pilot study (Story et al., 2003) and the Dietary Intake subsection of the Dietary Behavior scale from the National Youth Risk Behavior Survey, High School Version (CDC, 2009).
